Utah ESA laws and the exemption question
Before arguing about documentation, a Utah renter should settle a prior question: is this building covered at all? Utah has a meaningful set of exemptions, and knowing which side of the line you are on changes your entire strategy.

Is it a single-family home rented directly by its owner?

Is it operated by a religious organisation or private club?
Special rules apply. Utah has a significant amount of religiously affiliated housing, and these situations need checking individually.
Is it student or university housing?
Usually covered, and run through a campus accessibility process with its own forms and deadlines. Start earlier than you think you need to.

“We’re exempt” is a claim, not a fact
Landlords assert exemptions far more often than exemptions actually apply. Advertising the unit, using a letting agent, exceeding the unit threshold, or owning several properties can all change the answer.
If a Utah landlord claims an exemption, ask them to state the basis in writing. Requiring them to articulate it is frequently enough to establish that it does not hold.

Why the first four stop
Each exists because the lease treats the animal as a pet. Approval removes that characterisation, and the charges that depend on it fall away with it. Salt Lake City and Provo buildings that stack all four are simply applying a policy you are no longer inside.
Why the last two do not
Both arise from the tenancy rather than the animal. Utah landlords keep the security deposit and every ordinary remedy for damage, and no accommodation has ever changed that.
Expiry, renewal and out-of-state documentation in Utah
Utah Code 57-21 contains no expiry provision, so nothing about your letter lapses by operation of law. The practical deadline is set by housing providers, and Utah’s is the common twelve-month one.
