The Ohio Civil Rights Commission, and ORC 955.43

The OCRC enforces Ohio's fair housing provisions alongside HUD. If an Ohio landlord refuses a documented accommodation request, you have a state investigator and a federal one available, and using one does not close the other.

Separately, Ohio Revised Code 955.43 governs assistance dogs and penalises misrepresenting an animal as one. That statute targets fraud, but its practical effect on honest handlers is that a verifiable document now carries more weight than it used to.

Not as a matter of Ohio law. The Civil Rights Act contains no expiry clause and ORC 955.43 concerns access, not documentation shelf life. What exists instead is a convention, and conventions decide real cases.
